What parts of a chemotherapy treatment plan must be entered into eviti|Connect for preauthorization?
All parts of a chemotherapy treatment plan, including anti-cancer drugs and supportive drugs, must be entered into eviti| Connect for pre-authorization EXCEPT:
  • Delivery Systems
  • Medical Devices
  • Port Flushes
  • CAT Scans
  • PET Scans
  • MRI
  • Surgical Procedures
  • Chemotherapy drugs used to treat a non-cancerous diagnosis such as AIDS, arthritis, etc.
Do I need a complete set of chemotherapy orders to find a regimen in eviti?
Yes, you must enter all drugs to find a matching regimen. This will give you the best chance for receiving an automatic eviti code and reducing the need for additional treatment plans in eviti. For example, a common breast cancer regimen is Adriamycin and Cyclophosphamide followed by Paclitaxel. To find the matching regimen in eviti, you need the orders for both Adriamycin and Cyclophosphamide as well as the Paclitaxel. By entering all of the drugs associated with each phase of treatment, you would most likely receive an automatic eviti code for the entire treatment plan.

Please check with your clinical contact to make sure that you have received orders for a complete treatment plan rather than a partial treatment plan. The Chemotherapy Checklist, available on your eviti Home Page can be completed by your clinical contact to make sure you have complete orders.
